Model Guide Implementation Checklist

This Implementation Checklist for the Child Find ACCESS Model is used by leadership teams to track progress on implementation of the model components, identify areas of need, and determine next steps for implementing each component.

The checklist can be used at any phase of model implementation. The leadership team should revisit the checklist periodically. At a minimum, complete it once during each phase and additionally when there are meaningful changes to any aspect of the child find system or the team’s work together.

The checklist outlines essential elements and progress markers within each model component. It is designed to be completed as a group conversation.
